The Position- To provide care and treatment for patients in the comfort of their residence, primarily on campus with occasional off campus visits.
- Test and evaluate a patient’s abilities.
- Prepare individualized treatment plans and patient progress reports.
- Conduct therapies to improve physical, mental and psychological impediments and assist patients in improving the quality of daily living.
- One-year experience working with older adults in a long-term care setting or home health preferred.
