About the job:

The Registered Nurse under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team.

  • Graduate of a school of professional nursing.
  • Current state Registered Nurses (R.N.) License
  • Cardio Pulmonary Resuscitation (CPR) certification required
  • A minimum of one year’s experience in working with the frail or elderly population required
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Preferred Qualifications:

  • Bachelors of Science Nursing (BSN) preferred.
  • Conducts an in person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in person nursing re-assessment semi-annually and as needed.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • As needed provides orientation, initial and annual competencies, of, Licensed Practical Nurse (LPN), Certified Nursing Assistant’s (C.N.A.’s), and Clinic Clerk.
  • Performs competencies on appropriate day center staff as needed.
  • Performs general nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards.
  • Triages participants to determine priority of care.
