RoleThe Registered Nurse (RN) Case Manager provides, coordinates, teaches, and supervises the nursing care of hospice patients and their families or caregivers. Working in collaboration with other members of the interdisciplinary team, the RN Case Manager plans and manages the nursing component of the hospice caseload. The RN Case Manager makes initial and ongoing nursing assessments, judgments, and treatment decisions based on patient and family/caregiver needs and wishes, and in accordance with her/his professional skills.
Qualifications- Current RN license in the state of employment.
- Minimum of one (1) year of nursing experience; hospice or hospital experience preferred.
- Experience with an EMR/EHR system.
- Valid driver’s license and automobile insurance (Mobile Driver).
- Reliable transportation for home visits.
- Ability to use equipment with visual and auditory mechanisms.
- Effective communication skills in English (verbal and written).
- Physical ability to perform essential functions and requirements (lifting patients and/or equipment, bending, pushing/pulling, kneeling).
- Active BLS certification for healthcare professionals from the American Heart Association or Red Cross.
- Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT), integrating psychosocial, spiritual, cultural, physical, and biological components.
- Coordinates the Plan of Care and ensures it accurately reflects the patient’s evolving needs.
- Educates patients, families, caregivers, and other health professionals about disease process, palliative interventions, caregiving, and safety practices.
- Conducts home visits to assess safety, medication compliance, nutritional compliance, and durable medical equipment (DME) use.
- Reports changes in the patient’s condition to appropriate IDT members or other health professionals.
- Participates with the IDT in evaluating hospice referrals/admissions for level of care appropriateness.
- Attends daily IDT collaboration meetings and presents concise oral and written reports.
- Supervises patient care provided by Community Health Workers and Home Health Aides as requested.
- May be required to report to alternate work locations during emergencies and potentially stay overnight.
- Performs other duties as assigned.
Functioning requires sight, hearing, speech, movement, and use of hands. Frequent tasks include standing, sitting, reaching, lifting, bending, balancing; occasionally pulling, pushing, stooping, kneeling, and climbing stairs.
Compensation Pay Range$64,139.90 – $96,210.40.
This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as ongoing compliance with the Drug‑Free Workplace Policy.
