Position Summary

This position reports directly to the Program Specialist. The Community Integration Specialist will provide the support necessary for the persons we serve to achieve their desired lifestyles and increase levels of independence, productivity, and social/physical inclusion into the community. This includes developing and assisting with the person‑centered planning process, transporting persons supported to and from home and inclusive community activities, and supervising and implementing daily program activities and community experiences.

What You'll Be Doing Day-to-Day
  • Preparation for Change: involves planning for change, listening to individuals' interests, teaching essential community skills, providing support for success, and creating or reviewing Person-Centered Plans (PCPs) to ensure alignment with their preferences and goals.
  • Community Integration and Participation: helps individuals identify and plan community experiences based on their needs and interests, implements Person-Centered Plans for inclusive activities and skill development, provides support for community engagement, teaches life skills, fosters positive work habits, and ensures flexibility, including transportation, while promoting safety, dignity, and inclusion.
  • Personal and Health Care: assists individuals with daily living activities, provides constant supervision, ensures personal and health care needs are met, maintains a safe environment, responds to medical emergencies, administers medication accurately, adheres to safety protocols, and reports incidents, while working in a respectful, professional manner and following all policies and procedures.
  • Communication and Documentation: completes accurate documentation, attends required meetings and trainings, supports policies and the agency’s mission, promotes teamwork and person-centered planning, actively participates in Person-Centered Plan development, manages challenging behaviors, and maintains effective communication with team members, all while ensuring compliance with regulations and agency standards.
  • Managerial Duties: provides daily staffing support at various locations, instructs individuals in work-related skills, serves as a community liaison, ensures professional and respectful communication, and safeguards individuals' rights and dignity.
  • General: accurately documents attendance, submits behavioral data for review, completes regulatory reports on time, and maintains proper documentation in the Electronic Health Record System for daily logs, PCPs, and Behavioral Management Plans.
  • Must have a high school diploma or equivalent and be 18 years of age with a desire to work with adults who require a great deal of hands‑on assistance, personal care and supervision.
  • Must be computer literate and have excellent oral and written skills.
  • Must have strong communication skills.
  • Must be dependable and reliable.
  • Must have basic computer skills and be able to use all required software.
  • Must have the ability to problem-solve and assist in decision-making, including during emergencies.
  • Must maintain current status regarding assigned core competencies and other required certification acquisition within established schedules and timelines.
  • Must meet the criteria to be an approved driver for the Arc of Prince George's County and have reliable transportation.
  • Must have the ability to work productively with individuals and other team members to develop relationships.
  • Must possess excellent oral, written, and interpersonal skills and good customer service skills.
