General Description
Join our team and mission at the Wyoming Life Resource Center (WLRC) in Lander. WLRC operates an Intermediate Care Facility and a newly constructed Skilled Nursing Facility and is excited to build our team of CNAs. WLRC has adopted the Greenhouse model of care which is based upon the values of, "Real Home", "Meaningful Life" and "Empowered Staff." As part of the State of Wyoming, Department of Health, our team members enjoy a favorable schedule of twelve-hour shifts with three or four days off work each week and an excellent compensation and benefits package.

Essential FunctionsThe listed functions are illustrative only and are not intended to describe every function which may be performed at the job level.
- Assists with the development of policy, protocols, and procedures for implementing specific programs; orients new staff to the programs, policies, and procedures.
- Performs nursing care, assessment, diagnosis, treatment, and evaluation for residents.
- Provides guidance and oversight regarding nursing functions as programs are developed and implemented.
- Mentors, advises and guides staff nurses toward program outcomes. Provide support and guidance and maintain positive relationships among the healthcare team.
- Serve as Trainer. Manage care plans, provide new employee orientation, and staff in-servicing on new and current care plans.
- Provide feedback to the nursing department related to cottage changes, resident changes, and care management.
- Comply with facility and state standards of conduct.
- Attend team and house meetings. Serve as a member of the Inter-Disciplinary Team (IDT). Attend and provide expert knowledge of care plans and medical management of residents.
- Communicate and collaborate with providers, nurses, residents, family members, and multi-disciplinary team members.
- Assist with resident behavior episodes.
- Complete all required documentation and program charting in an accurate and timely manner.
- Takes responsibility for assigned special projects from initiation through project completion.
- Assist residents with off-campus appointments, confirm scheduling, and consent.
- Act as an advocate for the residents in communicating their needs, self-expression, and goals.
- Arranges for and conducts nursing clinics and classes, when assigned.
- Participate in rotating calls with other Senior Nurses to provide coverage for nurse call-ins.
- Other duties, as assigned.
Completion of WLRC internal Greenhouse training and completion of Lippincott requirements related to mental health and psychiatric health is required within six months of the date of employment in this position.
Knowledge- Knowledge of principles of nursing and management processes.
- Knowledge of medical diagnosis and patient care using nursing diagnosis.
- Knowledge of organization, planning, and team building.
- Knowledge of principles and practices of public health nursing.
- Advanced skills and knowledge in creating nursing diagnoses and nursing care plans.
- Knowledge of Intermediate Care Facility (ICF) and Skilled Nursing Facility (SNF) regulations, the Nurse Practice Act, and state and facility policies and procedures.
- Ability to recognize gaps and barriers that may occur and problem-solve to ensure resident needs are met.
- Ability to teach, mentor, and skills in communication and organization.
- Ability to assist with creating schedules that provide adequate nursing coverage.
- Ability to accept personal responsibility and accountability for decisions made daily.
- Ability to delegate duties/tasks and recognize when further consultation/assistance is needed.
- Skills in positive, accurate, and effective communication with all stakeholders.
- Ability to develop nursing diagnoses and plans of care.
- Knowledge of and ability to consistently use and demonstrate basic principles of infection control.
- Ability to provide nursing care to residents with challenging behavioral issues and intervene during a behavioral crisis.
Education: Bachelor's Degree (typically in Nursing).
Experience: 1-2 years of progressive work experience (typically in Nursing) with acquired knowledge at the level of a Nurse.
Certificates, Licenses, Registrations: Must be licensed or eligible for licensure as a Registered Nurse (RN) in the State of Wyoming.
OR Education & Experience Substitution: 4-6 years of progressive work experience (typically in Nursing) with acquired knowledge at the level of a Nurse. Must be licensed or eligible for licensure as a Registered Nurse (RN) in the State of Wyoming.
Physical Working Conditions- Ability to safely move (twisting, bending, kneeling, pushing, pulling), position and lift people with multiple and severe disabilities.
- Ability to monitor resident sounds, noises, location, etc. in order to ensure safety and assess imminent danger and emerging concerns.
