Job Summary/Purpose

The Hospice Registered Nurse is responsible for planning, coordinating, providing and documenting care for an assigned caseload of patients from day of admission through day of discharge. Provides nursing care to the terminally ill Hospice patient as needed, utilizing physicians' orders and professional skills, and implements a home health plan of care that meets each patient's specific needs.

  • Acts as a member of the interdisciplinary group team. Provides nursing care to patients using the nursing process (assessment, planning, implementation, and evaluation). Directs and leads other assigned team members and collaborates with multidisciplinary team members to provide age/developmentally appropriate care in accordance with nursing standards of care and practice. Provides patient/family centered care using the nursing process and focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs.
  • Maintains a safe, therapeutic, and healing environment through ensuring patient privacy and responsive, respectful, accepting, and professional behavior. Demonstrates ability to function in a professional setting through active participation in a professional practice model. Participates in shared governance, adherence to standards of care, participation in peer review and professional development promoting leadership and clinical excellence, effective team skills, autonomous practice, and acknowledgment of accountability for actions and critical thinking.
  • Coordinates the effective and efficient delivery of patient care services through the application of care management principles. Identifies clinical priorities and initiates action to improve patient outcomes.
Qualifications
  • Associate's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required. Bachelor's Degree preferred.
  • Prior leadership experience preferred. One year of case management and/or utilization review work experience preferred. Case Management certification preferred.
  • Licensure as a registered nurse by the state of South Carolina or compact state required.
  • Current Basic Life Support (BLS) or ACLS required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
  • Certification in Case Management preferred.
Physical Requirements
  • Ability to perform job functions while standing, sitting, walking, climbing stairs, working indoors and outside in temperature extremes, from elevated areas, in confined/cramped spaces, in kneeling positions, flexing at waist, squatting, pinching operations, gross motor activities with fingers and hands, firm grasping, fine manipulation, reaching overhead, repetitive motions, using lower extremities for balance and coordination, reaching all directions, lifting and carrying 50 lbs.

    unassisted, lifting/lowering objects 50 lbs. from floor, lifting from 36" to overhead 25 lbs., exerting up to 50 lbs. of force.
  • Ability to maintain 20/40 corrected vision, recognize objects close at hand or at a distance, match or discriminate colors, determine distance/relationship between objects, depth perception, good peripheral vision, maintain hearing acuity, perform gross motor functions with frequent fine motor movements, deal effectively with stressful situations, work rotating shifts, overtime as required, work in latex safe environment, maintain tactile sensory functions, maintain good olfactory sensory function, be qualified physically for respirator use.
