Overview
We are seeking a highly skilled LPN or RN to join our home health agency as a Private Duty Nurse for an adult client in Lancaster, Ohio on a part-time basis (Weekends).
This position must be filled immediately.

- Immediate / Starting Schedule = 24 Hours/wk
- Required shifts: Saturdays and Sundays (12 hrs both days) with flexible start and end times (approximately 6–8 am start / 6–8 pm end).

- 1+ years of paid experience in trach, G-tube care, and Trilogy/vent care, as well as medication administration and feedings
. - Ability to start part-time (24hrs/wk) and build to additional hours over time (if needed, but not required).
- This adult client has a loving, devoted family with both parents also serving as caregivers, so on-site support is available.
- The patient requires total care and support for all daily living needs, including bathing, dressing, feeding, assistance with routine maintenance exercises and passive range of motion, transfers, and other incidental services.
- Autonomy in a positive, client-centered culture, with excellent quality and compliance support plus training for all caregivers and clinicians.
- No on-call requirements.
- Eligible for OT if desired.
- Mileage reimbursement is provided for travel between multiple clients, if applicable.
- Full-time employees are eligible for medical and other company benefits.
- LPNs: $28/hour
- RNs: $35/hour
A successful Private Duty Nurse (PDN) at Home Care Network, Inc. performs skilled nursing functions in accordance with applicable laws and the guidelines of the professional organization and agency. All functions are performed in accordance with established policies and practices and the state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Clinical Administrator/Director of Nursing
- Graduate of an accredited school of professional nursing.
- Current license to practice as an LPN or RN in the state of Ohio.
- Minimum of one (1) year of experience in a home healthcare setting, acute care setting, or equivalent nursing experience is required. A passion for providing excellent in-home care is a must.
- Experience in trach care, G-tube care, medication administration and tube feedings required.
- Active CPR and First Aid certificates highly desired (agency will provide recertification if required).
- Good interpersonal communication skills - written and verbal.
- Current driver's license, good driving record, and reliable transportation.
- Desirable character traits: reliable, honest, professional, compassionate, detail-oriented (in care and documentation), accountable, friendly and accommodating, respectful.
- Excellent attendance record with past employers.
- Provides professional nursing care under the direction of a Registered Nurse and communicates status/condition changes to the RN per agency guidelines.
- Provides skilled nursing visits for clients as directed by the RN Case Manager and follows the care plan established by the RN.
- Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
- Communicates information on a timely basis, with documentation in the clinical record as required by policy and urgency of the situation.
- Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within the professional scope of practice as delegated by the Case Manager.
- May perform venipuncture for laboratory sampling if licensed as applicable (e.g., LPN M-IV).
- Promotes personal safety and a safe environment for clients and coworkers.
- Demonstrates knowledge of safety and infection control practices by compliance with policies and procedures.
- Recognizes and responds appropriately to potentially unsafe situations and uses equipment safely.
- Participates in improving safety and adheres to agency policies and professional standards.
- Performs duties in compliance with state and federal laws and agency standards.
- Maintains confidentiality and professional conduct; attends required meetings; supports quality improvement initiatives.
- Performs other related duties as deemed necessary.
Frequent standing, walking/bending/stooping; occasional sitting. Weight handling: up to 10–20 lbs frequently, 20–30 lbs infrequently; carry/lift up to 10–30 lbs infrequently.
