Position: Certified Medical Assistant- Population Health (Full Time)

Overview

The medical assistant is responsible for the care and treatment of clients in a medical office as directed by a physician. They work under the supervision of the provider, to ensure that quality care is provided to adult and pediatric patients.

Qualifications:

Graduate from an accredited school of medical assisting with knowledge of new trends and techniques in medical assisting. Certified or Registered Medical Assistant preferred; BLS required. Previous experience is also preferred.

Responsibilities
  • Patient Outreach & Engagement:
    Conduct outreach to patients for chronic disease management and preventive services. Assist patients with healthcare navigation and access to community resources.
  • Care Gap Closure:
    Utilize population health dashboards to identify and address care gaps in coordination with providers and nursing staff.
  • Chronic Disease Support:
    Monitor patient registries and follow protocols for disease management, documenting outreach and follow-ups accurately in the EHR.
  • Data & Documentation:
    Enter patient data into EHR/population health platforms. Assist with chart reviews and report on outreach activities and outcomes.
  • Quality Improvement Support:
    Participate in quality improvement projects and help implement workflows to improve clinic performance measures.
  • Clinical Support (Within MA Scope):
    Assist with standing orders for labs and provide patient education on self-management tools.
  • Social Determinants of Health (SDoH):
    Conduct screenings, document needs, and coordinate referrals to community resources, ensuring closed-loop referrals.

    Collaboration:

    Collaborate closely with care team members, participating in huddles and meetings to support patient outcomes.
