overview
clinical coordinator – stroke program
this position works under the direction of the stroke program manager to support the needs and operations of the stroke program. The clinical coordinator is responsible for oversight and coordination of clinical care provided to patients in the program during the hospital stay and up to 90 days upon discharge. Utilizes the stroke program’s electronic database to collect and track patient data concurrently and retrospectively to support quality and performance improvement endeavors.
Assists and supports routine and special functions of the stroke program, as needed.
- oversight and coordination of patient care
- maintains the current list of active patients in the program via the electronic database. Conducts concurrent review of patients within the program through their hospital stay, reviewing plan of care for compliance with standards and clinical practice guidelines.
- identifies variations or gaps in care and communicates deficiencies to involved care providers to ensure compliance with standards.
- assists and participates in the coordination of patient care, performs patient assessments and nursing interventions as needed, communicates with members of the care team as needed to ensure continuum of care. Performs psychosocial support with patients and families, as needed.
- review medical records and other source documents to collect clinical data of patients in the stroke program according to the specifications and guidelines of the applicable data element. Any fall-outs must be discussed with the stroke program manager and respective ed leadership and dedicated stroke unit’s leaders.
- keeps stroke program manager and data coordinator informed of patient status and reports any unusual or trending variances to support active and timely performance improvement. Assists the stroke program manager with concurrent review and coordination of patient care within the program as necessary.
- performs and documents patient education and goal setting as per the program’s current process. Oversees and evaluates education process concurrently to ensure compliance with standards.
- attends code stroke events in the hospital and acts as a resource and support during the process to ensure timely assessments, interventions, and standards of care are met. Ensures established specific order sets are utilized on patient diagnosis and presentation.
- collect and track patient data
- assists with collection of stroke data and entry into the electronic database(s) to include data required by regulatory agencies and data used for internal program monitoring and process improvement. Includes participation in data analysis and processes to ensure data validity and integrity.
- remain up-to-date with data definitions as related to abstraction of the data.
- perform timely call-back surveys for patients discharged from the program and enter responses into the electronic database. Perform education reinforcement as needed and referral of concerns or complaints to the appropriate party. Call-backs completed 1 to 2 days upon discharge, in 30 days, and 90 days on patients requiring mrs rankin.
- assist and support routine and special functions

- review any potential fall-outs with identified staff and provide real-time re-education.

- completes required continuous training and education, including department specific requirements

- refrains from disclosing confidential information acquired in the course of work, except when authorized and unless legally obligated to do so
- refrains from using or appearing to use confidential information acquired in the course of work for unethical or illegal advantage either personally or through third parties
- follows administrative policies on confidentiality and privacy practices
- other duties as assigned by the stroke program manager
