Relative Support Worker – Antelope Valley Partners for Health

Reports To: Program Manager Hours: Full‑time, occasional evening and weekend work required.

Job Summary: Provide supportive services to relative or NREFM caregivers who have primary care of children residing with them. The Relative Support Worker works closely with the program supervisor and program manager to manage the unique needs of each client.

Responsibilities
  • Have thorough knowledge of program goals, objectives, policies and procedures.
  • Know at‑risk, high‑risk families experiencing domestic violence, substance abuse, depression, and/or child abuse.
  • Contact families/caregivers within 48 hours of referral to assess needs.
  • Meet with clients to review needs, services and goals; inform them of voluntary services, rights, and child abuse reporting mandates.
  • Develop service plans with goals to identify strengths and address areas of need and concerns.
  • Document and file all contacts with families/caregivers within 24 hours of contact.
  • Use outreach to ensure participation and completion of service goals per grant requirements.
  • Travel within the city of Lancaster to provide services as needed.
  • Work closely with DCFS staff to create a comprehensive network of supportive services.
  • Utilize cultural competencies to establish trusting, nurturing, non‑judgmental relationships.
  • Assist families to strengthen safety, problem‑solving and coping skills; help utilize community services; advocate for needed services.
  • Make or assist families with referrals to community agencies.
  • Complete charting and documentation accurately and timely.
  • Meet training requirements and meet deadlines for multiple tasks.
  • Perform other duties as assigned.
Skills & Abilities
  • Knowledge of departmental policies, procedures, and organizational relationships.
  • Understanding of English language structure, composition, and grammar.
  • Knowledge of office practices, processes, and computer software (Excel, Word, Outlook).
  • Scheduling and coordinating skills.
  • Ability to work remotely/virtually.
  • Ability to work independently with minimal supervision.
  • Project planning and complex, long‑term project management.
  • Prioritization and organization of work.
  • Adherence to safety rules and confidentiality of client information.
  • Compliance with safety rules, regulations, and quality assurance guidelines.
  • Support the values and mission of AVPH.
Education & Experience
  • Bachelor’s degree preferred or minimum of 32 completed units from an accredited university or community college, with at least 16 units in Social Work, Social Sciences, Humanities, Public Administration or Public Health.
  • Two (2) years’ experience in social work, behavioral science, or social sciences.
  • Experience working with culturally diverse communities and demonstrating cultural sensitivity.
  • Ability to collaborate with multidisciplinary teams.
  • Excellent written and oral communication skills.
  • Detail orientation and accuracy.
  • Proficiency in Microsoft Excel, Word, Outlook; ability to learn new data programs.
  • Attention to detail and error‑correcting skills.
  • Bi‑lingual in English/Spanish preferred.
  • Covid‑19 vaccine highly encouraged.
Licensure & Certifications
  • Current CA driver’s license.
  • Current auto insurance.
  • Own transportation for work‑related purposes.
  • Current and maintained infant and adult CPR certification.
  • Meet basic health requirements, background check, and physical job standards.
  • Must have TB clearance.
Working Conditions
  • Flexible settings between office, remote/virtual, and in the community.
  • Work with overburdened, high‑risk families of varied cultural and economic backgrounds in high‑risk areas.
  • Maintain HIPAA requirements and confidentiality across various settings.
  • Position requires lifting/carrying up to 25 lbs., stooping, bending, squatting, sitting on the floor, walking (may be needed).
