Base pay range$26.00/hr - $32.00/hr
Duties- CASE MANAGEMENT:
Provides general case management and skill building for clients and their families in accordance with program policies and referring/regulatory agencies (DMH, DCFS, Probation, schools). - OBJECTIVES:
Develops measurable case management and individual rehab objectives for clients as outlined in the Client Care Plan and monitors progress toward those goals. - PRODUCTIVITY:
Completes thorough and timely documentation of all contacts with clients and maintains productivity standards set forth by the agency Tier System and internal policies. - MEETINGS:
Attends all staff meetings, in-service training and conferences as required and scheduled. - CONSULTATION:
Regularly consults with minor’s parent or guardian, probation officer, or CSW, and school staff. - TREATMENT TEAM:
Meets regularly with other members of the treatment team concerning client and program issues and coordinates needed services and linkages. - CONFIDENTIALITY:
Maintains strict confidentiality over all records and information relative to minors. - RESPECT:
Demonstrates respect for the intrinsic value of each person, their individual differences and culture. - CRISIS:
Ensures the safety of youth. Provides direct crisis intervention and therapeutic guidance. - CLIENT INTEREST:
Stimulates client’s interests in various clinical offerings. Monitors progress and maintains close contact with client and family. - BOUNDARIES:
Establishes positive, respectful and nurturing relationships with clients and families within professional boundaries and in accordance with agency policies and procedures. - TRANSLATION:
Provides translation services as needed. - STRENGTH-BASED:
Is capable of accurately identifying and building on the strengths of clients to better reach their potential. - TRAVEL:
Travels independently to various environments (buildings with stairs, various communities, schools, etc.) to provide services as needed. - OTHER:
Performs other duties as assigned by supervisor.
