Title: Physical Therapist – Skilled Nursing Facility (SNF)
Location: Lancaster, CA
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 – 16:30)

Job Responsibilities and Required/Preferred Skills & Requirements:

• Provide physical therapy services to patients in a Skilled Nursing Facility setting

• Evaluate, plan, and implement individualized treatment programs to improve mobility, strength, balance, and functional independence

• Treat patients with orthopedic, neurologic, and post-operative conditions including joint replacements, spinal conditions, stroke, Parkinson's disease, traumatic brain injury, and spinal cord injuries

• Perform assessments related to body mechanics and functional movement

• Develop and educate patients and families on home exercise programs

• Document evaluations, progress notes, and treatments using facility-approved charting systems

• Collaborate with interdisciplinary teams to support patient-centered rehabilitation goals

• May float to other care settings as needed (acute care, outpatient, rehab hospital)

Required Qualifications:

• Active Physical Therapist license

• BLS certification

• Minimum 1 year of experience preferred (new graduates will be considered)

• Experience or interest in Skilled Nursing Facility (SNF) setting

• Ability to manage a diverse rehabilitation caseload and adapt treatment plans accordingly

Preferred Qualifications:

• Prior SNF experience

• Experience with orthopedic and neurologic rehabilitation populations

• Strong patient and family education skills
