About the Role

We are seeking a Registered Dental Assistant – Community Dental Health Coordinator (CDHC) to join our Oral Health Services team. This unique position is designed to improve access to care for patients who face barriers such as language, culture, transportation, or financial limitations. The CDHC plays a vital role in patient education, outreach, and care coordination, with a strong focus on integrating dental health into overall medical care.

This role works closely with the Chief Dental Officer and interdisciplinary medical teams to address the oral health needs of patients living with chronic conditions such as Type 2 diabetes and HIV. The ultimate goal is to promote oral health as an essential part of total wellness while supporting patients in overcoming barriers to care.

Key Responsibilities
  • Enroll in and complete the CDHC Online Curriculum (tuition covered).
  • Partner with the Chief Dental Officer to design and implement the CDHC program model.
  • Identify and track patients with uncontrolled diabetes (A1C >
    9) or HIV requiring dental engagement.
  • Develop patient engagement and navigation processes to improve treatment adherence.
  • Create culturally appropriate and multilingual educational materials on oral health.
  • Track and analyze program outcomes using i2i population health dashboards.
  • Present progress and outcomes quarterly to leadership using data-driven reporting.
  • Coordinate with physicians, diabetic case managers, nutritionists, and pharmacy staff to ensure medical-dental integration (MDI).
