Salary and Hours£36,363 – £46,142 per annum | Part time 22.2 hours per week, 12‑month fixed term contract.
DescriptionA qualified and experienced Social Worker with empathy and a drive to make a difference will support people to achieve what they want and live as independently as possible in a place they consider home.
Responsibilities- Conduct strength‑based, outcome‑focused assessments and support planning to ensure residents receive the right support.
- Support people to access local services, maintain independence, promote health and wellbeing, and live independently.
- Collaborate with other agencies, professionals and informal carers to provide a holistic service.
- Work in line with the Mental Health Service’s values and the “Living Better Lives” vision.
- Engage with the community to drive forward innovative practice.
- Registered with Social Work England (SWE) or applying for registration.
- Experience in community or mental health social work.
- Valid driving licence and ability to use a car for duties; disability consideration possible.
- Will comply with DBS disclosure under the Rehabilitation of Offenders Act 1974.
- Strong interpersonal, communication and organisational skills.
- Up to 32 days annual leave, 8 bank holidays and 2 additional statutory days during the festive season (plus option to purchase).
