The Tissue Viability teams across MWL have an exciting opportunity to join the Tissue Viability Service with 2 Band 6 positions available across the sites, a substantive WTE 1.0 (37.5hr) Band 6 based primarily at Southport and Ormskirk sites and a substantive WTE 0.4 (15 hr) based primarily at Whiston, St Helens & Newton sites although some cross site working may be required.

Will provide a specialist role under the guidance and leadership of the Lead Tissue Viability Nurse. They will provide expert advice and clinical input into the prevention of pressure ulcers and management of complex wounds. They will support in the delivery of wound care and training of healthcare professionals including the supervision of clinical practice. They will work as part of the MDT with the aim of preventing admission, supporting early discharge and providing on going support and advice for patients with wounds within the acute setting.

They will provide advanced standards of clinical assessment, evidence based advice and education to prevent and manage wounds to patients, carers and healthcare professionals. They will undertake all aspects of clinical duties as an autonomous practitioner and represent the lead practitioner when they are not available. The JD & PS are currently under AfC review and are subject to change.

Clinical

& Professional Responsibilities
  • Maintain Professional Registration.
  • Adhere to relevant Code of Practice of Professional body.
  • Manage own workload and co-ordinate investigations and procedures necessary for treatment plans.
  • Assess, plan, deliver and evaluate individualised specialised programmes of care and to provide support and advice to other staff as appropriate.
  • Provide specialised wound management, planning, advice and guidance on a range of relevant topics to influence the practice of all health care professionals within the Trust and externally as/if required.
  • To use specialist knowledge to ensure liaison and/or referral to other members of the multi-disciplinary team.
  • Provide advice to staff in respect of appropriateness of equipment required in relation to assessed patient need.
  • To provide clinical supervision to health care professionals in relation to prevention of pressure ulcers/wounds and use of equipment for this purpose.
  • To be professionally and legally accountable for all aspects of own work including management of patients in their care.
  • To organise the job role effectively to manage clinical priorities and use of time.
  • Act as a patient advocate through the application of critical and professional knowledge and skills.
  • To communicate effectively and work collaboratively with the multidisciplinary team to ensure a co-ordinated service. This will include representing the Tissue Viability Team at service meetings, case conferences and corporate governance meetings.
  • Provide advice to palliative care services in wound and equipment management as appropriate.
  • Promote better understanding on the prevention and management of wounds to patients, carers and their families to encourage self-management through education and support.
  • To represent the organisation in regional and national forums as the expert on wound care and pressure ulcer prevention.
Administrative Responsibilities
  • Adhere to local and national recordkeeping guidance to ensure patient safety and accurate recording of patient information.
  • Ensure digital record keeping for wound care is undertaken with informed patient consent and completed daily after each patient consultation.
  • Active involvement in the TVN service clinical governance agenda by assisting the Lead TVN as required with the implementation of quality assurance programmes, education programmes, risk management procedures.
  • To provide monthly teaching and training for all staff in the prevention of pressure ulceration.
  • To provide teaching and training for link nurses on a bi-monthly basis to improve nursing standards in the prevention of pressure ulceration.
  • Responsible for keeping own clinical practice up to date with continuing professional development and revalidation.
Research & Audit
  • The post holder is required to participate in clinical and other local and national audits as required, including developing, setting and monitoring evidence-based care standards, protocols, guidelines and research projects for the TVN service.
