The Tissue Viability teams across MWL have an exciting opportunity to join the Tissue Viability Service with 2 Band 6 positions available across the sites, a substantive WTE 1.0 (37.5hr) Band 6 based primarily at Southport and Ormskirk sites and a substantive WTE 0.4 (15 hr) based primarily at Whiston, St Helens & Newton sites although some cross site working may be required.

Will provide a specialist role under the guidance and leadership of the Lead Tissue Viability Nurse. They will provide expert advice and clinical input into the prevention of pressure ulcers and management of complex wounds.

They will support in the delivery of wound care and training of healthcare professionals including the supervision of clinical practice.

They will work as part of the MDT with the aim of preventing admission, supporting early discharge and providing ongoing support and advice for patients with wounds within the acute setting.

They will provide advanced standards of clinical assessment, evidence based advice and education to prevent and manage wounds to patients, carers and healthcare professionals.

They will undertake all aspects of clinical duties as an autonomous practitioner and represent the lead practitioner when they are not available.

The JD & PS are currently under AfC review and are subject to change.

Responsibilities
  • Managing workload and coordination of investigations and procedures necessary for treatment plans.
  • Assess, plan, deliver and evaluate individualized specialised programmes of care to provide support and advice to other staff as appropriate.
  • Provide specialised wound management, planning, advice and guidance on relevant topics to influence the practice of all healthcare professionals within the Trust and externally as/if required.
  • Use specialist knowledge to ensure liaison and/or referral to other members of the multidisciplinary team.
  • Provide advice to staff in respect of appropriateness of equipment required in relation to assessed patient need.
  • Provide clinical supervision to healthcare professionals in relation to prevention of pressure ulcers/wounds and use of equipment for this purpose.

Key Duties
  • Ensure that the care of patients with wounds in the acute setting is to a high standard and support the Lead Nurse Specialist to ensure the clinical governance agenda includes evidence-based protocol development, audit, risk management and professional development.
  • Co‑ordinate the care of patients, carers and their families with wounds within the acute setting to provide a seamless service within the multi‑professional team, including staff from other statutory and voluntary agencies, both hospital and community based.
