The RoleAs a Social Care Support Officer, you will provide strengths-based, outcome-focused support to adults in the community. Your role is all about having meaningful conversations, listening to what people want to achieve, and working collaboratively to create personalised support plans that promote independence and wellbeing.
You’ll work alongside other professionals, families, and community groups to ensure people receive the right support at the right time. By focusing on what people can do, rather than what they can’t, you’ll help prevent or delay the need for formal care and support, enabling individuals to stay connected to their communities and live life their way.
Key Responsibilities- Carry out strengths-based assessments and reviews, putting people’s wishes and aspirations at the heart of everything you do.
- Work collaboratively with health colleagues, families, carers, and community organisations to support safe and timely discharges from hospital and promote independence at home.
- Help people access local resources, voluntary groups, and community support, encouraging them to stay well and connected.
- Support people to regain life skills through short-term interventions, avoiding unnecessary long-term care.
- Promote creative and flexible use of personal budgets to meet individual needs.
- Keep accurate records and contribute to service development, always looking for ways to improve how we work.
- Uphold the wellbeing principle of the Care Act 2014, ensuring every intervention supports people’s dignity, autonomy, and independence.
- A positive, person-centred approach and a genuine desire to help others.
- Great communication skills and the ability to work well in a team.
- Commitment to equality, diversity, and safeguarding.
- IT literacy and confidence in managing records.
- Willingness to learn and develop through ongoing training and support.
