Senior Urgent Care Practitioner
Join the team as a permanent band 7 Senior Urgent Care Practitioner at Hywel Dda University Health Board and support the Ceredigion Same Day Urgent Care/Outreach service.
OverviewAn exciting opportunity to be part of a team dedicated to ensuring the best outcomes for patients. The service is constantly developing in line with local and national healthcare provision, and we seek a forward‑thinking, competent, and caring practitioner to triage, assess, and treat patients while working closely with ACPs and the community multidisciplinary team (MDT).
Responsibilities- Perform in‑depth advanced assessments and develop care plans with community, primary and acute MDT members.
- Carry out treatments, triage and assess patients, and manage workload effectively in a fast‑paced environment.
- Collaborate with ACPs and the community MDT to improve patient integration across care settings.
- Work shift patterns covering 7 days a week, day shifts 08:00‑20:00 and night shifts 19:30‑08:30.
- Band 7 Senior Urgent Care Practitioner with experience of autonomous practice in emergency/urgent/MIU settings.
- Broad understanding and experience of acute and chronic conditions.
- Strong diagnostic skills and the ability to multi‑task.
- Excellent clinical and communication skills, calm and confident approach to work.
- Commitment to service improvement and willingness to embrace change.
