Overview
Join KWA (Korean Women's Association) as a Full-Time Homecare Referral Coordinator in Lakewood, WA. This onsite position focuses on referral management, screening & assessment, and coordinating with Well Sky and CRM tools to enhance reporting and analytics.
Responsibilities- Centralized intake, tracking, and follow-up of all homecare referrals from diverse sources.
- Ensure timely, professional, and coordinated responses to referral sources and prospective clients.
- Maintain accurate and comprehensive records of all referral activity.
- Collaborate with regional teams to maximize opportunities, advance patient/client advocacy, and streamline case management processes.
- Provide strong customer service and apply an empathetic approach to service delivery.
- Experience with homecare referral systems and homecare contracting in Washington State.
- Understanding of Long-Term Services and Supports (LTSS) and community-based care programs.
- Associate degree in healthcare administration or social services is required; bachelor’s degree preferred.
- Two years of experience in healthcare, homecare, or case management intake coordination.
Pay range: $22.00–$24.00 per hour. Benefits include Medical, Dental, Vision, 401(k), Life Insurance, Paid Time Off, Paid Training, and disability coverage. 11 Paid Holidays and 401(k) eligibility after 1 year.
