• Provide hands‑on care in the home for a group of patients, consistently using the nursing process and clinical standards and protocols.
• Oversee the activities of the multidisciplinary team that provides care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review patient visits weekly and provide a plan to the Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of any schedule changes and communicate caseload and patient care issues to the Home Care Supervisor.

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License – State of Florida (Florida driver's license is required within 30 days of hire)

• Required:
  
  Associate's or Diploma in Nursing
• Preferred:
  Bachelor of Science in Nursing

• Required:
  
  1 year as a Registered Nurse
• Preferred: 1 year in Home Care
