• Home visits in assigned service area:
  Polk County

• Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

• Certifications and Licensures: BLS (Basic Life Support), LPN (Licensed Practical Nurse) License, Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• Education:
  
  Associate's or Diploma Nursing
• Experience:
  
  1 year Licensed Practical Nurse, Preferred 1 year Home Care
