We are seeking an experienced Home Health Physical Therapist (PT) to provide skilled in-home therapy services to patients across multiple clinical conditions. This role involves conducting comprehensive patient assessments, developing and implementing individualized care plans, and delivering high-quality rehabilitative services in a home health setting.
The Physical Therapist will work independently, manage daily visit schedules, and ensure accurate clinical documentation while promoting patient safety, functional mobility, and recovery.
Key Responsibilities- Perform comprehensive physical therapy evaluations and assessments
- Provide skilled therapy for cardiopulmonary, neurological, and orthopedic patients
- Develop and implement individualized Home Exercise Programs (HEP)
- Educate patients and families on safety, mobility, and injury prevention
- Assess and train patients on use of mobility equipment and assistive devices
- Monitor patient progress and update care plans accordingly
- Identify signs and symptoms of physical, psychological, or sexual abuse and report as required
- Perform wheelchair assessments and ADL training
- Ensure accurate and timely clinical documentation (OASIS preferred)
- Maintain compliance with Medicare, Medicare Advantage, and Commercial insurance requirements
- Complete 6–8 home visits per day (minimum productivity expectations apply)
- Participate in weekend coverage (1–2 weekends per contract)
- Use provided laptop, MiFi, and portable printer for documentation
- Med/Surg
- Cardiac, CHF, COPD, Respiratory
- Orthopedic (hips, knees, shoulders, post-surgical)
- Neurological (CVA, Parkinson’s)
- Trauma, amputations (AKA/BKA)
- Falls, weakness, failure to thrive
- Tracheostomy patients
- Minimum 1 year of Physical Therapist experience
- Home Health experience required
- OASIS experience preferred
- Strong clinical assessment and documentation skills
- Experience with gait training, mobility aids, and transfer devices
- Excellent patient and family education skills
- Knowledge of safety, injury prevention, and abuse recognition
- Ability to work independently and self-schedule visits
- Active State Physical Therapist License
- BLS Certification (required)
- Valid Driver’s License and reliable vehicle
- COVID vaccination (or approved exemption)
- Flu vaccination (or declination with mandatory mask use)
- Must be comfortable with self-scheduling based on patient needs
