• The Home Health PT provides hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy.
  
  Responsibilities include:
• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient s needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patient visits weekly and provides plan to PCS and team assistant, including anticipated weekend visits.
• Informs team assistant of any schedule changes and communicates caseload and patient care issues to the Patient Care Supervisor.

• BLS (Basic Life Support)
• PT (Physical Therapist) License
• Driver s License State of Florida (Florida driver s license is required within 30 days of hire)
