The Home Hospital RN reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. The Registered Nurse embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of the department leadership, consistently reflects the mission, vision, and values of NM, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Home Hospital Nurse treats acutely ill patients in the home as a substitute for in-hospital care. The Home Hospital Nurse is a forward-thinking, creative individual who wants to practice in an innovative clinical setting and values interdisciplinary teamwork and patient-centered care. The Nurse will provide hospital-level acute care in a patient’s home and will be part of the NM Hospital at Home team including (depending on each patient’s needs) physicians, paramedics, advanced practice providers, pharmacists, case managers, social workers, home health aides, physical therapists, and other operational staff.
Provides home visits within defined radius around site hospitals according to Hospital at Home care model.
- Care Coordination:
Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation. - Communicates changes in patient’s condition promptly.
- Uses appropriate nursing diagnosis and data from the assessment to derive realistic outcomes and documents them as measurable goals on the plan of care; modifies care plan as indicated.
- Identifies and prioritizes patient needs and/or nursing diagnoses and documents.
- Collaborates with patient, family, and other members of the health care team when planning patient care and documents accordingly. Assesses discharge planning needs and makes appropriate referrals.
- Evaluates and documents patient response to nursing interventions and adjusts the plan of care as necessary.
- Assesses discharge planning needs and makes appropriate referrals, including palliative care.
- Ensures completed discharge instructions, medication reconciliation, and provides information to patient/family regarding the use of the patient portal and other resources.
- Patient Education/Ethics and Diversity:
Supports patient/family/spiritual/cultural needs; assesses teaching readiness and documents education records. - Utilizes teach-back for medication education; distributes Krames patient education.
- Incorporates Culture Vision in care planning and delivery; treats patients, families, and staff with respect and dignity.
- Acknowledges and values contributions of patient’s significant others as part of the plan of care; adheres to ethical standards of practice; calls for ethics consult when appropriate.
- Evidence-Based Practice (EBP) and Research:
Implements nursing interventions based on the established plan of care and evidence-based care; uses reliable sources to advance nursing practice. - Demonstrates involvement in clinical ladder projects or unit/organization process improvements.
