Overview
The salary range for this position is $34.50 - $55.20 (Hourly Rate). Placement within the salary range is dependent on factors such as relevant work experience and internal equity. For union-represented positions, placement is guided by the collective bargaining agreement rules. The Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under department leadership, the nurse reflects NM s mission, vision, and values, adheres to the Illinois Nurse Practice Act, the Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory and accreditation standards.
Responsibilities- Care Coordination:
Performs comprehensive assessments on assigned patients and documents findings; communicates changes in patient condition promptly. - Utilizes nursing diagnosis and assessment data to establish realistic, measurable goals in the plan of care; updates the plan as needed.
- Identifies and prioritizes patient needs and nursing diagnoses; collaborates with patient, family, and health care team; assists with discharge planning and referrals as necessary.
- Evaluates patient response to interventions and adjusts the plan of care accordingly; ensures discharge instructions and medication reconciliation are completed; provides patient/family with information about the patient portal and resources.
- Supports patient/family teaching needs, documents education, and uses teach-back for medication education; distributes Krames patient education materials; uses culture vision in care planning.
- Values patient and family contributions; adheres to ethical standards; requests ethics consultations when appropriate.
- Implements interventions based on an established plan of care and evidence-based practices; uses resources such as Ovid/Joanna Briggs, Up to Date, Culture Vision, and other evidence sources; engages in unit/organization improvement efforts.
- Demonstrates safe medication administration with bar coding; protects patient health, safety, and rights; provides timely, accurate bedside hand-offs and rounds with the care team; participates in quality metrics and safety initiatives; supports preventing pressure ulcers, falls, CAUTIs, and central line infections as applicable.
- Oversees and directs the work of patient care or support staff; demonstrates effective interpersonal skills; serves as a resource and role model for new staff and students; participates in precepting, orientation, and mentoring; engages in peer evaluation and interdisciplinary collaboration; contributes to daily unit operations and quality initiatives.
- Required:
Bachelor of Science in Nursing (BSN);
Current State of Illinois Registered Professional Nurse;
Basic Life Support; strong interpersonal and collaboration skills; strong analytic abilities; ADN with a plan to obtain a BSN within three years of employment - Preferred:
National Certification in area of specialty
