Social Worker PNE LCSW-Palliative Care Part Time Days
Northwestern Medicine Catherine Gratz Griffin Lake Forest Hospital – Part‑time (20 hours per week) role in Palliative Care.
Salary range: $34.62–$50.20 per hour. Placement within the range depends on relevant experience and internal equity.

DescriptionThe Social Worker reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Social Worker is responsible for providing psychosocial counseling and intervention services to patients who request their services, or any patient referred by a provider. The social worker advocates for the patient and family and assists in outpatient continuity of care.
Responsibilities- Complete a thorough psychosocial assessment for new patients.
- Utilize critical thinking to identify and screen high‑risk social cases.
- Provide pre‑treatment planning, psychosocial counseling/interventions, health education, financial counseling, referrals, and discharge/transition planning.
- Collaborate with team, other ancillary departments, case management staff, and social service agency providers to develop a care plan.
- Provide resources, including education materials, to patients and families in the areas of financial assistance, transportation, scheduling, and community resources to support the care plan.
- Support department initiatives (attend case conferences, participate in quality improvement).
- May perform other duties as assigned.
- Master’s degree.
- Licensed Clinical Social Worker (LCSW) in the state of Illinois.
- Zero to two years’ experience in a relevant role.
