Position: Registered Nurse-Special Care Nursery Full Time Nights Sign On Bonus Eligible

Overview

The salary range for this position is $34.50 - $55.20 (Hourly Rate). Sign-on bonus available for this position: $3,000. Placement within the salary range is dependent on factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules in the collective bargaining agreement.

Description

The Registered Nurse embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Under the direction of department leadership, the nurse consistently reflects the mission, vision, and values of Northwestern Medicine, adheres to the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and related regulatory and accreditation standards.

Responsibilities
  • Care Coordination
    • Performs a comprehensive assessment on all assigned patients with complete documentation.
    • Communicates changes in patient condition promptly.
    • Uses nursing diagnosis and assessment data to set realistic outcomes and measurable goals on the plan of care; modifies the plan as needed.
    • Identifies and prioritizes patient needs and documents them.
    • Collaborates with patient, family, and health care team; assesses discharge planning needs and makes appropriate referrals.
    • Evaluates patient response to interventions and adjusts the plan of care as necessary.
    • Assesses discharge needs and refers to palliative care when appropriate; ensures discharge instructions and medication reconciliation are completed and provides information to patient/family about the patient portal and resources.
  • Patient Education/Ethics and Diversity
    • Supports patient/family needs for teaching; documents education and learning readiness.
    • Uses teach-back methodology for medication education.
    • Utilizes Krames and Culture Vision resources for education and care planning.
    • Respects cultural and ethical standards; involves family as appropriate; seeks ethics consult when needed.
  • Evidence-Based Practice (EBP) and Research
    • Implements nursing interventions based on the established plan of care and evidence-based practice.
    • Maintains professional nursing standards and uses reliable sources to inform care.
    • Demonstrates involvement in unit/organization-level improvement projects.
  • Healing Safe Environment
    • Demonstrates safe medication administration with barcode verification.
    • Promotes patient health, safety, and rights; communicates clearly at bedside hand-offs.
    • Rounds hourly with the patient care technician; maintains a safe environment and contributes to quality metrics.
