RN Case Manager - Hospice role based in Lake Forest, California. Join our client's dedicated team in Lake Forest, CA, as an RN Case Manager for hospice care. You'll coordinate and advocate for patients' care, ensuring they receive comprehensive and compassionate services in their homes.

• Conduct intake assessments with patients and families.
• Develop individualized care plans considering physical, financial, and other constraints.
• Coordinate patient care with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust treatment plans as needed.
• Ensure patients have access to necessary healthcare services and resources.
• Educate patients and their families about managing illnesses and treatment plans.
• Advocate for patients  needs with insurance providers and healthcare facilities.

• Certification: Licensed Registered Nurse with no restrictions.
• Education: Bachelor s degree in nursing required; advanced degree preferred.
• Experience: At least 1 year as an RN Case Manager or similar role in hospice care.
• Skills: Includes strong interpersonal and communication skills; ability to work independently and as part of a team; proficiency in patient care documentation and management.

• Pay: $85,000 - $100,000 annually
