Salus Hospice is urgently seeking a compassionate and skilled Hospice RN Case Manager/Admissions to join our dedicated team serving the Riverside County region. Hospice RN Case Manager Responsibilities- Perform RN duties on hospice team.
- Serve as a RN Case Manager for the hospice Interdisciplinary Team.
- Assess for signs and symptoms of discomfort including presence of pain and take prompt and appropriate action.
- Establish a plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate.
- Coordinate all hospice services provided to patients/families in all settings to ensure quality of care.
- Registered Nurse with valid CA state license.
- Minimum one year of hospice RN experience.
- Strong background in medical/surgical, ICU/CCU, or ER is helpful.
- Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient residence.
- Must have excellent interpersonal skills and a passion for hospice.
- Understanding of hospice philosophy and needs of the terminally ill.
- Self‑directed with ability to work with little supervision.
- Current Physical and TB test results.
