Active LPN license in LA

Current BLS certification

Hospice or palliative care experience preferred

Strong communication and clinical assessment skills

Responsibilities:

Provide compassionate bedside nursing care to hospice patients

Administer medications and treatments as prescribed

Monitor patient comfort and symptom management

Document care in EMR accurately and in a timely manner

Communicate with the care team and families as needed

Maintain compliance with hospice standards and HIPAA

Respond promptly to patient needs and end-of-life concerns

Perform other duties as assigned
