Transfer Center/Admissions Registered Nurse

Department: Nursing Administration

Location: Lake Charles, LA

DESCRIPTION OF POSITION:

  • The Registered Nurse provides professional nursing care for patients within an assigned unit of a hospital, in support of medical care as directed by medical staff and pursuant to objectives and policies of the hospital. They are responsible for total nursing care for all patients assigned to him/her by the Nurse Manager and/or Charge Nurse.

RESPONSIBILITIES AND DUTIES:

  • Effectively prioritize direct admissions from physician offices/nursing facilities and direct admissions transfers from other hospitals.
  • Provides effective communication to patients/clients, their family members, team members, and other health care professionals to ensure efficient patient transfer.
  • Demonstrates knowledge of common diagnoses requiring transfer.
  • Accurately interprets clinical information (labs, imaging, vitals, etc.)
  • Identifies level of care needed (ICU, telemetry, med-surg, etc.)
  • Applies knowledge of EMTALA and state/federal/health system transfer regulations
  • Coordinates with house supervisor, patient access, and ED department staff
  • Documents calls and communication accurately
  • Demonstrates assertiveness and professionalism
  • Uses SBAR style reporting effectively
  • Identifies and resolves barriers to transfer
  • Uses clinical judgment to guide transfer appropriateness

EDUCATION AND WORK EXPERIENCE

  • Education:

    Graduate of approved School of Nursing
  • Experience:

    2 years acute care experience: ED experience preferred
  • Technical

    Skills:

    As required by department/unit
  • License/Certification:
    Current Louisiana Registered Nurse license or Multi-state Compact License.

Physical Demands/Work Environment

  • May require walking long distances, standing for prolonged periods, bending, and lifting 50-100 pounds at a time.
  • May require pushing and/or pulling patient in wheelchair.
  • The work environment may be physically confining.
  • Many nursing skills require acute hearing ability, i.e. listening with stethoscope to breath sounds and heart sounds.
  • Must be able to exchange accurate information with patient, family, peers and medical personnel.
  • Establish rapport with families, patient, and healthcare team.
  • Respect and value cultural differences.
