As a Hospice Admissions Nurse, you will be responsible for executing the hospice admission process in compliance with federal and state regulations. You will advocate for the patient and family to meet their needs that hospice can cover and refer them to other resources as appropriate.
Primary Responsibilities- Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needs
- Maintains confidentiality and safety of patient/client clinical records
- Completes assigned admissions and patient care orders in an effective and timely manner
- Communicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient. Records that may be relevant to determine hospice eligibility
- Communicates effectively with operational, clinical and sales leadership regarding status of pending referrals
- Current unrestricted RN licensed in the state of operation
- 2+ years of experience in a hospice, home health, or other healthcare service delivery system setting
- Current CPR certification
- Current Driver’s License and vehicle insurance, and access to a dependable vehicle or public transportation
- LA: a minimum of two years full‑time experience working as an RN. However, two years of full‑time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse
- Demonstrated fiscal responsibility and accountability
- Proficient computer skills with knowledge of Microsoft Word and Excel

The hourly pay for this role will range from $28.27 to $50.48 per hour based on full‑time employment. We comply with all minimum wage laws as applicable.
