Overview

Public Health Registered Nurse 1, 2, or 3 (3 vacancies). Covering Butts, Carroll, Coweta, Fayette, Heard, Henry, Lamar, Meriwether, Pike, Spalding, Troup, and Upson*

Perinatal Health Partnership (3 vacancies).

Location:

District 4 Public Health, 301 Main Street, Lagrange, GA 30240. Home base will be determined upon hire. Travel will be reimbursed according to state travel regulations. In some cases, agency vehicle may be made available. Daily travel is a requirement for this position.

Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Functions as a team providing case management services to high-risk perinatal women, infants, and children through home visitation and in collaboration with health care providers and other human service agencies to provide person-centered support to individuals and families; to help improve access to care; assist with navigating the health care and social service system;

support research through outreach and advocacy and health needs assessments; advocate for individual, family and community needs; and build client capacity to increase knowledge and self-sufficiency. Provides outreach, support, and education to the target population and their surrounding community to reduce disparities in birth outcomes.

Qualifications and responsibilities may include travel throughout the district’s 12 counties for training and program support as needed.

Job Responsibilities
  • Primarily performs home visits and completes required documentation per the Perinatal Health Partnership guidelines to eligible women, infants, and children
  • Provides community health and case management services to high-risk perinatal women, infants, and children primarily through home visitation
  • Provides outreach, support and education to the targeted population and their surrounding community
  • Consults and collaborates with other professionals involved in providing services for women, infants, and children
  • Assesses patient’s condition to develop plan of care and provide treatments, intervention, and follow-up
  • Provides follow-up for physician’s plan of care and recommendations; assists patients, hospital staff, and physicians with care coordination
  • Assists families in establishing goals and outcomes
  • Participates in community collaboration meetings in assigned counties
  • Provides patient education and referrals to assist in attaining targeted goals and outcomes
  • Records pertinent observations and response to treatment
  • Incorporates strategies for interacting with persons from diverse backgrounds
  • Prepares to respond to public health emergencies and respond as directed
  • Assists with completing monthly/quarterly reports
  • Uses individual, team, and organizational learning opportunities for personal and professional development
  • Candidates must have a valid, unexpired driver’s license and reliable transportation
  • Travels throughout the district’s 12 counties for training and program support as needed
Minimum Qualifications
  • PH Registered Nurse Level 1 - Associate's degree in Nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. No experience required.
  • PH Registered Nurse Level 2 - Bachelor’s degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification; OR Associate's degree in Nursing AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Nurse license AND CPR certification.
  • PH Registered Nurse Level 3 - Bachelor’s degree in Nursing from an accredited college or university AND One year of nursing experience AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification; OR Associate’s degree in Nursing AND Two years of nursing experience AND completion of a PHN course within One year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
