Under broad supervision, provide assistance to public health clinics throughout the 12 counties within District 4 Public Health. Assigned to specific clinical duties within the scope of LPN practice in public health. Act as a lead worker over specified projects or programs.
All District 4 Public Health, County BOH employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Distribution site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.
- Primarily performs home visits and completes required documentation per the Perinatal Health Partnership guidelines to eligible women, infants, and children
- Provides community health and case management services to high-risk perinatal women, infants, and children primarily through home visitation
- Provides outreach, support and education to the targeted population and their surrounding community
- Consults and collaborates with other professionals involved in providing services for women, infants, and children
- Assesses patient ’s condition to develop plan of care and provide treatments, intervention, and follow-up
- Provides follow-up for physician’s plan of care and recommendations; assists patients, hospital staff, and physicians with care coordination
- Assists families in establishing goals and outcomes
- Participates in community collaboration meetings in assigned counties
- Provides patient education and referrals to assist in attaining targeted goals and outcomes
- Records pertinent observations and response to treatment
- Incorporates strategies for interacting with persons from diverse backgrounds e.g., cultural, socioeconomic, educational, racial, gender, age, ethnic, sexual orientation, professional, religious affiliation, mental and physical capabilities
- Prepares to respond to public health emergencies and respond as directed
- Assists with completing monthly/quarterly reports
- Uses individual, team, and organizational learning opportunities for personal and professional development
- Candidates must have a valid, unexpired driver’s license and reliable transportation
- Travels throughout the district’s 12 counties for training and program support as needed
Annual salary - $33,000
Vocational/Technical degree in licensed practical nursing from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse AND CPR certification AND completion of all necessary orientation and training for Public Health LPN. No experience required.
PH Licensed Practical Nurse Level 2Annual salary - $35,800
Vocational/Technical degree in licensed practical nursing from an accredited college AND Two years of LPN public health experience AND current Georgia license to practice as a Licensed Practical Nurse AND CPR certification.
PH Licensed Practical Nurse Level 3Annual salary - $38,926.79
Vocational/Technical degree in licensed practical nursing from an accredited college AND Four years of LPN public health experience AND current Georgia license to practice as a Licensed Practical Nurse AND CPR certification.
Note:
Level of LPN position hired will be based on number of years performing nursing duties consistent with Public Health nursing experience.
