• Excellence in Patient Experience by partnering with health care team, patient and family.
• Patient Centered Care — Clinical Practice: support an environment of healing or wellness for patients, address patient needs, follow the plan of care as assigned or instructed by a licensed professional, engage patient and family in compliance with the treatment plan, and contribute to the plan of care with a focus on the care continuum.
• Consistently implement safety interventions to minimize risk in achievement of outcomes.
• Protect the rights and dignity of the patient and maintain confidentiality adhering to HIPAA regulations.
• Participate in hourly patient rounding and respond to telephone and call lights promptly and professionally.
• Resolves patient concerns or escalates to the charge nurse if unable to resolve them.
• Proactively offer assistance and comfort measures to patients and families; present self in a positive and professional manner.
• Provide patient care under the direction of the RN and perform needs-based tasks (e.g., hygiene, nutrition, elimination, physical comfort) and additional advanced skills as unit requirements (e.g., phlebotomy, EKG, removal of Foley catheters, IV removal).
• Assist RN and other health care team members with care delivery, assist in direct patient admission, monitor data entry for key patient safety information, participate in charge reconciliation, and perform audits as requested.
• Support unit performance improvement initiatives, transport unit supplies, patient care supplies, medications and equipment, and practice accountability.
• Maintain communications using written and oral skills with patients, staff, physicians, family and visitors; use safety communication techniques and chain of command when necessary.
• Document completion of patient care in the EMR and update patient information on white boards; stay informed as a unit expert regarding patient care documentation tools, especially during downtime.
• Provide a safe environment for patient care and follow infection control policies including PPE usage and universal precautions; ensure proper body mechanics and use assistive devices as required.

• High School Diploma General-Preferred or GED General-Preferred

• All certifications are required upon hire unless otherwise stated.
• BLS - Basic Life Support with the American Heart Association
• CNA - Certified Nurse Assistant - preferred

• Graduation from a State approved program for Certified Nursing Assistants or Patient Care Tech Program Upon Hire - Preferred

• Direct patient care experience - preferred

• Ability to follow detailed directions
• Strong interpersonal skills
• Clear verbal and written communication, specifically ability to document
• Ability to work individually and as part of a team
