Role Responsibilities:
- Lead discharge planning and coordinate post-hospital care for patients within 30 days after discharge, collaborating with a multidisciplinary team to ensure the best outcomes.
- Assess patient needs, determine appropriate levels of care, and facilitate smooth transitions to the next phase of recovery.
- Work with insurance providers to secure coverage and support patient education and preventive care programs.
- Play a key role in reducing readmissions and improving patient satisfaction through effective communication and planning.
- Requires an Associates of Nursing (ASN).
- Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program.
- Requires 3-5 years of relevant experience.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Requires proficiency in Microsoft Office and applications.
- Requires understanding of medical record requirements, regulations and policies.
