Job SummaryThe Hospice Registered Nurse Case Manager is responsible for planning, coordinating, delivering, and documenting care for an assigned caseload of patients from admission through discharge. Working under physician orders, the RN develops and implements individualized plans of care that meet each patient’s needs while ensuring compliance with agency policies, accrediting standards, and applicable federal and state regulations.
Responsibilities- Complete admission assessments and ongoing patient evaluations in the home setting.
- Develop, implement, and update patient plans of care in collaboration with physicians.
- Collect and document OASIS data accurately and on time.
- Provide skilled nursing care while managing multiple patients as assigned.
- Assess, manage, and educate patients and families on pain, medications, and disease processes.
- Coordinate care with physicians, interdisciplinary team members, and agency staff.
- Participate in case conferences and communicate changes in patient condition as needed.
- Maintain timely, accurate clinical documentation and comply with agency policies and regulations.
- Uphold patient safety, quality standards, and confidentiality at all times.
- Complete required trainings, meetings, and continuing education.
- Graduate of an accredited school of nursing.
- Active Registered Nurse (RN) license in the state of Indiana.
- Minimum of one (1) year of nursing experience in a hospital, facility, home health, or related healthcare setting.
- Home health experience preferred.
- Valid driver’s license, current auto insurance, and reliable personal transportation.
- Ability to work independently in patient homes and manage a flexible schedule.
- Ability to sit, stand, and drive for extended periods while providing in-home care.
- Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
- Regular use of a tablet for documentation, communication, and training.
