Occupational Therapist – Alternate Solutions Health Network (Redcrest Home Health)

Schedule:

Part‑time, 3 days per week; 8:15 am to 5:00 pm. Territory:
Lafayette, Crawfordsville, Wallace, Covington, Boswell, Fowler, Brookston, Monticello, Buffalo, Logansport, New Waverly, Galveston, Flora, Cutler, Frankfort, etc.

HOW YOU'LL WORK

You will provide comprehensive skilled services as ordered by the attending physician, educate patients and families, ensure patient safety, complete accurate documentation, and comply with all legal rules and regulations.

Major Areas Of Responsibility
  • Plan of Care – Provide skilled occupational therapy as ordered, record and report patient progress, develop and adjust individualized OT plans.
  • Evaluation – Apply diagnostic and prognostic procedures to evaluate patient functioning.
  • Patient Outcomes – Use activities and home exercise programs to improve range of motion, muscle strength, and coordination.
  • Minimize Patient Risk – Teach patients preventive measures to reduce re‑hospitalizations.
  • Family Educator/Advocate – Counsel patients and families, instruct proper use of equipment.
  • Supervision – Oversee and supervise total patient care provided by COTA.
  • Communication/Collaboration – Communicate with COTA on supervisory visit results.
  • Policies – Complete documentation following agency protocol and Medicare/Federal guidelines.
  • Rules and Regulations – Follow agency policies, procedures, rules, and regulations.
  • Operations – Attend in‑service training and mandatory agency meetings.

Requirements
  • Occupational Therapist with a current license in the state of employment
  • Minimum one‑year experience as an OT in an acute care setting; home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands
