Registered Nurse - Home Care

Our long‑standing home health and hospice client—trusted in the community for over 100 years—has a Home Care RN opening serving the Greater Hudson region.

• Complete initial home health assessments and full physical evaluations
• Plan, deliver, and evaluate skilled nursing care using the nursing process
• Ensure accurate OASIS scoring through comprehensive assessments
• Develop patient‑centered care plans with families included in decision‑making
• Manage your own caseload and play a key role in helping patients regain strength and independence

Required:

• RN from an accredited nursing program (Associate degree minimum)
• 1+ year of recent acute care experience (med/surg or acute rehab preferred)
• Strong clinical judgment and ability to work independently
• CPR certification
• Clean criminal and driving record (mileage reimbursed)

Preferred:

• BSN
• Community Health/Education background
• Additional clinical certifications (IV, Chemo, Ostomy, etc.)
• Familiarity with OASIS and reimbursement drivers
