Job Summary

Busy Family Practice looking for a motivated registered Nurse to assist our medical staff. The Nurse will provide support to the health, wellness, and disease management of Health First Patients.

• Responsible for nursing care at patient appointments
• Obtaining vital signs and patient history
• Provides individualized education to patients related to acute and chronic diagnosis
• Triages patient calls and schedules visits as appropriate
• Administers oral and injectable medication and treatments within the scope of practice and with orders from providers. May distribute sample medication per provider orders.
• May perform venipuncture blood draws and urine collection for lab work as ordered by the provider
• Maintains patient data and health information in the electronic medical record according to documentation guidelines
• Generates patient-related reports, as needed
• Participates in performance improvement programs and disease management programs
• Assists in the coordination of well-child and immunization programs
• Assists in ordering and stocking clinical supplies
• Completes transitional care management follow-up for patients with hospital admissions
• Daily monitoring of assigned Athena One Buckets for patient follow-up according to outline protocol, including patient portal messages
• Monthly participation on the Life Risk Safety Committee
• Routine safety and risk assessment to support Just Culture
• Support care coordination efforts as requested by the primary care provider

• Education/
  
  Experience:
  
  Graduate of a Nursing Education Program, Current unrestricted Registered Nursing License for the state of NH; BLS certification required. One year of nursing experience preferred.
