Concord Hospital Medical Group is seeking a compassionate and collaborative experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our
** Rheumatology
* * team. This full-time position offers a balanced opportunity to practice in both
** Concord and Laconia**, caring for patients across the Lakes Region and Capital Area of New Hampshire.

This role is ideal for an experienced APP who values patient education, team-based care, and developing long-term relationships with patients managing chronic rheumatologic conditions.
** About the Role
** As part of our Rheumatology practice, you will care for patients with a wide range of autoimmune and inflammatory conditions, including but not limited to rheumatoid arthritis, lupus, psoriatic arthritis, gout, and vasculitis. You’ll work closely with experienced rheumatologists and a multidisciplinary care team to deliver high-quality, coordinated care.
** Key Responsibilities
*** Perform comprehensive patient evaluations, including history, physical exams, and diagnostic assessments
* Manage and monitor patients with chronic rheumatologic conditions
* Develop and adjust treatment plans, including medication management and monitoring
* Review and interpret laboratory and imaging results
* Provide patient and family education on disease processes, medications, and lifestyle considerations
* Collaborate with physicians and other care team members to ensure seamless care
* Participate in quality improvement initiatives and clinical program development
** Qualifications
* ** Licensed Nurse Practitioner or Physician Assistant in New Hampshire (or eligible for licensure)
* Prior experience in rheumatology and internal medicine/primary care strongly preferred
* Strong clinical judgment, communication, and organizational skills
* Commitment to patient-centered care and education
