About the Position
This position is located in the Family Medicine Service, Lacey, Washington. Lacey, Washington offers a blend of suburban comfort, abundant green space, and easy access to larger cities—making it a popular choice for families, military personnel, retirees, and commuters.

This is a Direct Hire Solicitation.
Duties- Serve as Nurse for Population Health Improvement to promote a coordinated continuum of care and services.
- Manage the health metrics and outcomes of the primary care population.
- Liaise with health care providers, primary care team members, and Nurse Case Managers to identify beneficiaries with both short- and long-term health care needs.
- Conduct Quality Improvement (QI) studies to support process enhancements related to population health.
- Enhance adherence to population health metrics, reduce administrative burdens for DoD beneficiaries, and improve cost-effectiveness across the civilian–military health care system.
- Evaluate population health and medical management programs, interpret data, and provide evidence-based recommendations.
- Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
- A Personnel Security Investigation (Tier
1) with Child Care Background Investigation (CNACI) is required. - This position has specific initial background investigation and periodic re-verification requirements since its duties require interaction with children and youth under the age of eighteen (18) on a recurring basis.
- Selectees may be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
- This position is a Testing Designated Position subject to pre-employment screening and random drug testing. Selectees will be required to consent to participation in random drug urinalysis testing.
- A pre-placement medical examination may be required.
- Immunization screening may be required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
- Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
- Moderate to heavy lifting up to 50 lbs may be required.
- Position requires the employee to wear a uniform and/or protective clothing.
- This position is designated Weather Essential and required to work during severe weather closures.
- Applicants must be able to fulfill non-privileged professional credential requirements.
