Immediate Licensed Practical Nurse (LPN) - Home Health travel contract opportunity in Lacey, WA.

Note: LPN must have 1 year recent home health experience for a Medicare Certified Agency

Pay:

• $1,878.20 (Travel pay package)
• Local pay package available
• Mileage reimbursement for patient related travel

Work Schedule:

• 8:00am - 5:00pm (Monday to Friday)
• 40 hours per week
• No weekends
• 13-week contract
• Area of coverage is Lewis County - there may be crossover so we request flexibility with location

Job Details:

• Administer skilled nursing care and education under the supervision of registered nurse for adult clients in their place of residence, coordinate with the interdisciplinary team, patient/family and provider.
• Support patient recovery and ability to stay at home – enabling them to spend quality time with their loved ones and doing the activities they enjoy
• Empower patients and families to reach healthcare goals by educating them on disease management
• Case manage and provide clinical oversight to interdisciplinary team

Qualifications

• Current LPN License
• Must have 1 year recent home health experience for a Medicare Certified Agency
• Hands-on HCHB experience required
• Current driver's license
• American Heart Association BLS
