Overview

Social Worker MSW - Hospice job at Providence Health & Services. Lacey, WA.

Responsible for social work evaluation of patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Work is evaluated by Home Health Manager through patient record documentation, verbal communication and direct observation. Work is normally performed in a home setting. Requires considerable travel. Occasional work is performed in an office setting.

  • Perform social work evaluation and contribute to the development and implementation of the social work treatment plan.

  • Document in patient records in alignment with insurance guidelines and organizational standards.

  • Collaborate with the patient/family and interdisciplinary team to address psychosocial and environmental needs in home-based settings.

Required Qualifications
  • Master's Degree - MSW from a school of Social Work.

  • Washington Advanced Social Worker License upon hire. Or

  • Washington Advanced Social Worker Associate License upon hire. Or

  • Washington Clinical Independent Social Worker License upon hire. Or

  • Washington Clinical Independent Social Worker Associate License upon hire.

  • Within 30 days of hire:
    National Provider Identifier (NPI) number

  • 1 year of experience in a health care setting.

  • 3 years of Post Master's work experience, or prior experience in hospice care setting.

Preferred Qualifications
  • 2 years of experience in a health care setting.

  • Prior experience in home health/hospice care setting.
