Overview

The Welbe Health PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We’re driven to offer the best care for participants and provide a positive work environment for our team.

The Home Health RN provides skilled, compassionate care in participants’ homes under CMS guidelines, coordinating care plans with the interdisciplinary team (IDT) to empower participants to live healthier, more independent lives.

• Skip OASIS assessments and focus on patient care
• Hourly pay that includes travel time rather than pay-per-visit
• Work-life balance through flexible scheduling and built-in documentation time

• Conduct home health nursing assessments to determine nursing equipment needs in the home, participant preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document home health nursing interventions as agreed in care plans, including maintaining a healthy and safe environment, promptly responding to physician orders, administering medications, and performing ordered tests and treatments
• Instruct participants, families, and caregivers regarding disease processes, self-care techniques, and prevention strategies
• Track and monitor home health scheduling in collaboration with the Home Health Manager or scheduling contact
• Collaborate with the Physician and IDT to support smooth transitions between settings (hospital, skilled nursing facility, home, etc.)

• Graduate of an accredited school of nursing
• Unencumbered RN license
• Minimum of one (1) year of nursing experience in a clinical setting with frail or elderly populations; home health experience preferred
• Reliable transportation

Salary/Wage base range: $47.74 - $63.02 hourly + Bonus. Welbe Health offers a comprehensive total rewards package including 401k match, healthcare coverage, and other benefits. Actual pay will be adjusted based on experience and qualifications.
