Job Description Summary

In Charles County Public Schools, we believe that every student deserves the opportunity to achieve their full potential and lead fulfilling lives beyond the classroom. As a Transition Support Teacher for the Adult Career, Community, Education & Enrichment (ACE) Program, you will play a pivotal role in empowering students aged 18–21 who are working toward program completion. You will support their transition from school to adult life by fostering independence and preparing them for supported or competitive employment, community engagement, and lifelong learning.

You will be instrumental in supporting students’ attainment of the Maryland Certificate of Program Completion (MCOPC) Endorsements for Post-Secondary Education, Career & Work Readiness, and Community & Citizenship by developing and delivering targeted curriculum and instruction aligned to MCOPC competencies and individual transition plans. You will collaborate closely with ACE staff, students, families, and community partners to ensure each student has the tools and support needed to succeed after exiting the public school system.

EDUCATON,

QUALIFICATIONS AND WORK EXPERIENCE
  • Bachelor’s degree and meets certification requirements as established by the Maryland State Department of Education.
  • Experience teaching academic, social, functional, and behavioral skills to students with significant disabilities.
  • Knowledge of special education laws, regulations, and legal issues.
  • Knowledge of secondary transition services, including MSDE compliance requirements and evidence-based best practices.
  • Experience developing and implementing individualized transition plans for students moving from school to adult life.
  • Experience working with community services/partners such as DDA, DORS, DOL, Centers for Independent Living, Community Rehabilitation Providers (CRP), etc.
  • Evidence of exemplary human relations skills when working with diverse groups (parents, professionals, students, administrators, community agency representatives).
  • Evidence of strong organizational and planning skills.
  • Valid Maryland driver’s license with a clean driving record.
  • Ability to pass all CCPS transportation requirements.
ESSENTIAL JOB FUNCTIONS
  • Coordinates and monitors the implementation of secondary transition plans for ACE students aged 18–21, ensuring compliance with state and federal requirements.
  • Supports the documentation of MCOPC endorsement completion as part of students' transition plans.
  • Develops and adapts curriculum and instructional materials that align with MCOPC competencies and support individual student transition goals.
  • Implements transition-focused lessons in ACE classrooms targeting MCOPC competencies and secondary transition skills, including self-advocacy, work readiness, independent living, and community participation.
  • Conducts ongoing transition assessments and utilizes data to guide instruction and transition planning.
  • Collaborates with ACE teachers, para educators, related service providers, and other support staff to promote a team approach to transition services.
  • Coordinates student-centered transition activities and connects students with community resources to support post-school outcomes in employment, education/training, and independent living.
  • Ensures completion of transition interviews and transition documentation prior to IEP meetings.
  • Monitors and reports students’ progress on transition activities and MCOPC competency development quarterly.
  • Communicates effectively with families, community agencies, and adult service providers to ensure a smooth transition to post-school life.
  • Follows required procedures, including all Special Education legal procedures and guidelines as outlined in IDEA and COMAR.
