* Position Summary:

** We are seeking a compassionate and experienced
** Registered Nurse (RN)
** to provide
** weekend
* * home health services across the
** South County region**. This role involves planning, organizing, and coordinating care, with a strong emphasis on community health and patient education.
** Responsibilities:
*** Develop and initiate individualized plans of care in collaboration with patients, caregivers, and physicians
* Deliver preventative and rehabilitative nursing procedures as ordered
* Educate patients and caregivers on health management and care techniques
* Identify and implement discharge planning needs
* Coordinate necessary equipment and services for patient care
* Supervise LPNs/LVNs and Home Health Aides
** Requirements:
*** Active California RN license
* Minimum 2 years of direct patient care
* Minimum 1 year of home health experience required
* Current CPR certification
* Valid driver’s license and insured, reliable vehicle
