Position OverviewThe Lead MDS Coordinator provides leadership and oversight of the MDS process to ensure accuracy, compliance, and timeliness in accordance with CMS, state, and federal regulations. In addition to performing MDS Coordinator duties, this role serves as a subject matter expert, supports interdisciplinary collaboration, and assists in mentoring staff to ensure optimal resident care planning and reimbursement outcomes.
Schedule:
Monday to Friday
Pay Range: $48-52/hour depending on experience
- Serve as the primary lead for all MDS‑related processes and compliance
- Provide guidance, training, and support to nursing and interdisciplinary team members related to MDS completion
- Monitor MDS schedules and workflows to ensure timely and accurate submissions
- Assist the Director of Nursing with audits, surveys, and regulatory reviews related to MDS
- Coordinate and complete all required MDS assessments (admission, quarterly, annual, significant change, PPS 5/14/30/60/90‑day)
- Conduct resident assessments, evaluate responses to interventions, and document accurately in the EMR
- Complete CATS, CAAs, and care plans within CMS‑required timelines
- Ensure all triggered and non‑triggered conditions (medications, treatments, skin integrity, and risk factors) are reflected in resident care plans
- Identify changes in condition and determine the need for significant change MDS assessments
- Meet daily with Rehab to case‑manage PPS and HMO residents
Current, active California LVN or RN license
Minimum of 1 year of long‑term care or geriatric nursing experience
Prior MDS experience required
Leadership or supervisory experience preferred
