Brief Description

We are seeking either 1 full-time candidate or 2 part‑time candidates (1 per clinical location between the Health Center in La Grange Park and BEDS Plus in Summit). The Family Nurse Practitioner or Physician Assistant works under the direct supervision of the Chief Medical/Population Health Officer, who is also the collaborating physician. The FNP/PA completes all duties associated with their clinical license and manages patient care in accordance with the professional guidelines of Pillars Community Health.

This position is a collaborative role between Pillars Community Health and Beds‑Plus, with the objective of optimizing health for adults experiencing housing insecurity by improving access to primary care.

The FNP/PA spends 50% of their time providing medical assessment and care at the Summit Service Center—a dedicated location for individuals experiencing homelessness—and the remaining 50% at the PCH Health Center in La Grange Park. The FNP/PA instructs, oversees, and works alongside RNs and Medical Assistants in delivering care, supports the policies, procedures, and protocols of the Health Center, and collaborates with the Quality Improvement Specialist to ensure quality assurance.

They embrace clinical goals set by the Chief Medical/Population Health Officer, ensure accurate EHR management, and maintain HIPAA and OSHA compliance.

Essential Duties And Responsibilities Patient Care
  • Provides general medical care and treatment to patients under the direction of a physician and in accordance with Illinois Clinical License, PCH policies, and procedures.
  • Performs physical examinations and preventive health measures within prescribed guidelines to diagnose and treat acute, chronic, and long‑term health issues.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs.
  • Prescribes or recommends medication or other forms of medical treatment, including therapeutic procedures.
Case Management
  • May refer patients to physicians for consultation or to specialized health resources for treatment.
  • Coordinates patient treatment, management, and planning with clinical staff and the physician to provide continuity of care, including assistance with referrals to other agencies and health care providers.
  • Submits health care plans and goals of individual patients for periodic review and evaluation by the physician.
Patient Records
  • Responsible for documentation of patient care; records physical findings, and formulates plans and prognoses based on patient condition.
Patient Education
  • Provides education to patients and/or their families about preventive care, medical issues, and the use of prescribed medication and treatments.
Clinical Quality Performance
  • Assists in the compilation of statistical data and patient outcomes for agency needs, as requested.
  • Attends all meetings as required.
  • Completes encounter requirements as established by HRSA (Health Resources & Services Administration) for FQHCs (Federally Qualified Health Centers).
  • Adheres to OSHA guidelines for a safe work environment.
Professional and Staff Development
  • Works with students and interns to evaluate and care for patients.
  • Participates in continuing professional development to stay informed of clinical practices and incorporates this knowledge into practice.
Other
  • Maintains compliance with health center immunization requirements.
  • Performs other duties as assigned.
Qualifications

Education And Experience
  • Must hold an active and unrestricted Advanced‑Practice Nurse or Physician Assistant license in the state of Illinois, after completing a graduate‑level or higher nursing or physician assistant accredited program and obtaining professional certification.
  • Current CPR and DEA certifications and Medicaid and Medicare provider numbers required for credentialing and privileging.
