This is a full-time on-site role for a Physical Therapist, located in Kokomo, IN. The Physical Therapist will assess patients, develop personalized treatment plans, and provide therapy to improve mobility, alleviate pain, and enhance physical function. Daily responsibilities include conducting evaluations, maintaining detailed documentation, collaborating with interdisciplinary teams, and educating patients and caregivers on therapy techniques and practices.

• Strong skills in physical assessment, therapeutic exercise planning, and treatment implementation
• Proficiency in patient education and communication for effective therapy outcomes
• Experience in documentation, record-keeping, and collaborating within interdisciplinary teams
• Commitment to delivering patient-centered, empathetic, and culturally sensitive care
• Doctorate in Physical Therapy from an accredited program
• Active Physical Therapist license in the state of Indiana
• CPR and First Aid certification preferred
• Excellent organizational and time management skills
• Experience with geriatric patients or in a healthcare setting is a plus
